Skilled Nursing Care

Skilled nursing care is provided by a licensed nurse who has received special training and has experience in providing care in a home setting. The goal of nursing services is to help our patients feel better, function more independently, and learn how to better manage their health. Specific duties include:

• Medication management
• Wound care including wound VAC therapy
• Cardiac and congestive heart failure care
• Pain management
• Education and disease management 
• Pulmonary rehabilitation
• Diabetic care and monitoring
• IV therapy
• Central line and port maintenance
• Tube feeding
• Ostomy care
• Bedsore prevention and care
• Lab draws and reviewing of results
• Clinical assessments
